Here at Abertawe Medical Partnership we use an electronic system for transferring patient records to and from other surgeries, where possible, called GP2GP. This system enables patient records to be transferred directly and securely between GP practices. It improves patient care as GPs will usually have full and detailed medical records available to them for a new patient’s first consultation including: past medical history, information about medication, allergies, adverse reactions and immunisations. Once we receive your paper records we will also review this information to summarise and enter any appropriate information into our clinical system.
